How does My Dynacare Card Program work?
Under Marquette University’s PPO Health Plan, outpatient laboratory tests performed by Dynacare Laboratories are covered 100% when you use your My Dynacare Card. By using the card, you avoid paying co-pays, deductibles, and co-insurance for most outpatient laboratory tests.
How do I use My Dynacare Card?
When your health care provider requires lab tests, present your My Dynacare Card or your Medical
Insurance ID card. Using either of the cards means that Dynacare Laboratories will perform the lab tests here in Milwaukee.
Am I required to use My Dynacare Card?
Using your card is voluntary. Your regular health plan benefits will apply if you choose not to use it.
Who pays for the My Dynacare Card Program?
Marquette University, your Employer.
Where can specimens be collected for the My Dynacare Card Program?
You have several options:
• At your health care provider’s office, or
• At any of the 30 Dynacare lab specimen collection sites conveniently located throughout
Milwaukee and surrounding areas. Click here for a complete list of lab

What laboratory tests are covered by My Dynacare Card Program?
The majority of outpatient laboratory tests are covered under the program, including blood tests such as CBC’s and cholesterol; urinalysis; pap smears; biopsies; and culture testings. However, some lab testing is not covered, such as tests while you are hospitalized, specialized tests (fertility, bone marrow, and spinal fluid); lab tests not approved and/or not covered by your health benefit plan; and lab tests when you do not use your My Dynacare Card.
What if my health care provider does not use Dynacare Laboratories?
All health care providers are welcome to use the My Dynacare Card program. Feel free to give them Dynacare’s Client Service phone numbers above. On the website, select “Send My Physician Info”, complete the applicable information and we will contact your provider. Remember, you always have the option to go to any of the Dynacare lab specimen collection sites.
What should I do if I use My Dynacare Card and receive a bill?
First, you should review the explanation of benefits you receive to determine the reasons the lab testing was not covered. If you disagree with the reasons for the denial, contact UMR, your health benefit plan claims administrator.
